4. Policy owners must present their Manulife App displaying their AMILI reward to avail of the offer. Payment must be made directly by the policy owner to the participating clinic partners in Singapore Dollars.

5. Policy owners will need to return the sample by hand directly to the clinic or via SingPost (a mailbox service in Singapore) using  the pre-paid postage labeled envelope provided  in the kit. Any other courier services used by a policy owner to deliver samples to AMILI’s Lab will be at the policy owner’s  own expense. 

6. AMILI and its participating clinic partners do not offer kit collection or sample pick-up service outside of Singapore. If the policy owner is based outside of Singapore, the policy owner  must  collect the kits from AMILI’s participating partner clinics in person and arrange for sample delivery to AMILI’s laboratory independently. 

7. The package only includes one (1) session of  teleconsult report interpretation by a physician of the participating clinic. Additional consultation sessions requested by the policy owner with the physician will be at the policy owner's own expense.

8. This promotion cannot be combined with other discounts such as Senior Citizen and PWD discounts.
