LIFE POLICY
For change or correction of billing address, fill out the Personal Details Change form and submit to Client Services Department.
Request should be filed at least five (5) weeks prior to the next premium due date to facilitate sending of premium notice to the new address.
Fill out the upper portion of the Assignment / Release of Assignment form and submit to Client Services Department together with a Beneficiary Designation form. Accomplish the Irrevocable Form if you are assigning an irrevocable beneficiary/ies.
Court order will be required if the irrevocable beneficiary to countersign the beneficiary change form is a minor.
If you wish to update your beneficiaries on your policy, fill out the Beneficiary Designation form and submit to Client Services Department. Assign a trustee for minor beneficiary/ies and indicate the relationship to the Life Insured.
Accomplish the Irrevocable Form if you are assigning an irrevocable beneficiary/ies.
Please note that you are required to submit a Court Order if the irrevocable beneficiary to countersign the beneficiary change is a minor.
Letter of explanation may be required if the beneficiary being assigned has no insurable interest (e.g., friend).
Fill out the Application for Policy Change form and submit to Client Services Department.
For addition of rider, fill out the Statement of Insurability form also. Additional documents may be required depending on the initial declaration in the insurability form. Additional premium should be paid plus a policy fee of Php150.00 if the application for addition of rider is beyond six (6) months from Issue Date.
For deletion of rider, submission of court order is required if the irrevocable beneficiary to countersign the policy change form is a minor.
Fill out the Application for Personal Details Change form and submit to Client Services Department together with a photocopy of Birth Certificate.
Policy contract should also be submitted and premium arrears should be paid, if there is a change in age.
Fill out the Application for Policy Change form and the Application for Attained Age Conversion form and submit to Client Services Department together with your Policy Contract.
New premium and arrears should be paid.

There are two (2) types of Reinstatement namely:
Fill out the Statement of Insurability/Application for Reinstatement form and photocopy of valid identification form (ID) and submit to Client Services Department. Additional documents may be required depending on the initial declaration in the insurability form.

Fill out the Request for Surrender for Cash Value form and submit to Client Services Department together with the following documents:

A certification from the Assignee may also be submitted confirming that the loan has been duly paid. If the assignee is a bank or corporation, a secretary's certificate should be submitted together with the photocopy of valid ID of the signatories on the release letter/form.
In case your policy contract is lost or was torn out due to long period of storage or due to typhoon, you may request for a duplicate copy by filling out the Declaration of Loss of Policy form, have it notarized and submit to Client Services Department.
A policy fee of Php142.00 should be paid upon submission of the form.
Dividends left on deposit to accumulate interest can be withdrawn anytime. It can also be used to pay for premium due or to pay off existing loan. However, for policies with outstanding loan which exceeds the cash value, dividend withdrawal will not be allowed unless used to pay off the loan.
Fill out the Request for Dividend Withdrawal form and submit to Client Services Department together with a photocopy of your two (2) valid IDs and your policy contract.
There are four (4) modes of payment where you can choose from in paying your premium due.
Fill out the Request for Mode Change form and submit to Client Services Department. If you choose the monthly mode, please submit 12 postdated checks (PDCs) or a Credit Card Authorization form to Billing and Collection Department. To use the Automatic Debit Arrangement (ADA) facility, you must enroll your account with Chinabank.

Application for change in plan should be made within six (6) months from issue date.
Fill out and submit the Application for Policy Change form and Statement of Insurability form to Client Services Department together with your policy contract. Additional documents may be required depending on the data disclosed in the initial documents submitted. Additional premiums may also be required.
